Physical Therapy annually provides in-service presentations to caregivers in assisted living and independent living situations on safe transfers, fall prevention and skin breakdown.

Occupational Therapy obtains indicated assistive devices for home safety to help with independent performance of activities of daily living.

 Patient Care Coordinator Coverage

Our Patient Care Coordinators are essential tools to sucessfully initiate the continuity of care for our patients. They provide communication to Primary Care Physicians, family members, and patients to provide optimal transition for skilled services. Our Coordinators work hand in hand with our patients to ensure families and patients of services covered by their insurance.

What if I am admitted into the hospital…

Our Patient Care Coordinators are available to lend support in your recovery by resuming services upon your discharge. Simply contact our office or your Patient Care Coordinator. They will communicate with the patient, their doctors, and discharge planners to make certain the patient will continue services upon arrival home.

Additionally, the Patient Care Coordinator provides valuable services at no cost to the patient or facility. Services such as balance screenings, blood pressure clinics, exercise classes, arranging for equipment “tune ups,” as well as social events are provided throughout the month; with special events planned periodically through the year.
